Re: Accounting Office--Change in Hours of Service

Like most campus departments and programs, the current campus budgetary situation has struck the Accounting Office very hard. To cope with a reduction in staffing and continue to perform required accounting responsibilities, Accounting Office hours will be reduced, beginning Monday, August 9, 2004, to 9 AM to 3 PM. This change is one of several being made as part of larger reorganization of the Payroll Office that takes effect on August 2. Other changes include:

  • Payroll Office Staff Duty Changes: Payroll office staff duties will change. An updated list of Payroll Office staff duties and contact information is available at the following web address: http://www.ucsc.edu/finaff/payroll/payservices.htm. Service Centers, departmental staff members, and employees should contact the appropriate Payroll Office staff member directly.

  • Payroll Check Pick-up: Payroll and accounts payable checks may be picked-up from the Accounting Office Mail Clerk, Sylvia Brown, between 9 AM and 3 PM.

  • Stop payment and reissue of lost payroll check: The General Accounting section of the Accounting Office will process stop payment and reissue of both accounts payable and payroll checks. Contact information is available from the web address referenced above.

    • Duplicate pay stubs

    • Verifications of employment

    • Address changes

    • 403(b) plan enrollment

    • W-4 withholding tax status changes

Instructions for obtaining the personal identification numbers needed to access these websites can be obtained by selecting "Forgot PIN" on the UC At Your Service website or by selecting "Lost your PIN" under the "Sign in for Help" section of the UC for Yourself website. Both of these websites operate around the clock and may be accessed from work or home using an internet-connected computer equipped with the appropriate web browser software. A kiosk with access to UC for Yourself and UC At Your Service has been set up in the Accounting Office for use by UCSC employees. Other kiosk locations include the Benefits Office and McHenry Library.

  • Payroll-related Forms: The following payroll-related forms are available from the Accounting Office forms table:

    • SurePay enrollment form for direct deposit

    • Request to cancel Sure Pay direct deposit form

    • Request for stop payment and reissue of lost payroll check

    • Request for duplicate W-2 form

    • Address change form

    • Change of payroll check disposition

    • Request for cancellation of certain payroll deductions
