C A M P U S D I R E C T O R Y U P D A T E

The 2001-2002 UCSC Campus Directory is going into its early production phase and we need information from all faculty and staff regarding their directory listing. Individual Update Forms for the UCSC Campus Directory were distributed through the the mail in May. The information gathered through these forms appears in the Faculty and Staff listing in the back section of the Campus Directory.

If you have received the form, please review, update your information and return the form by 6/29/01 to CATS Direcory Services (27 Communications Bldg.). A timely submission will ensure accurate information for the new 2001-2002 UCSC campus directory and for our campus' 911 Emergency system.

If you are not listed in the current campus directory, or if you have not received a form through campus mail, please fill out the form on page 15 of the current directory, and send it in by the 6/29/01 deadline.

Should your unit receive an update form for an employee who has left your department, please forward to their current unit if they are still on campus. If you know that the employee has actually left the campus, please indicate on the form and return to CATS Directory Services, 27 Communications Bldg.
